How do I make an appointment?

Before you make an appointment to see Dr Alice Huang, you will need a current referral from your Doctor.

If you are making an appointment for fertility treatment or advice and you have a partner, your partner will also need a referral (or be named on your referral).

Once you have obtained your referral, please send it to us via email to [javascript protected email address] or fax it +61 3 9417 1099 or via post to 33 Victoria Parade, Collingwood Vic 3066. Please include your details so that we can contact you to make an appointment.

Please note: GP referrals are valid for 12 months and Specialist referrals are valid for 3 months.

What do I need to do before my first appointment?

Please complete and return your Patient Registration Form (one for yourself and one for your partner if applicable).

What will I need to bring to my first appointment?

Please bring any and all relevant medical reports. This includes (but is not limited to):

• Pathology reports (blood tests)
• Ultrasound reports (usually pelvis/gynaecological/abdomen/testes)
• Semen analysis report(s)
• Current medication details
• Medical history
• Medicare card/Proof of ID

If available, please send these details prior to your appointment. If you do not have copies, please request that a copy of any relevant reports/results from your previous treating doctors are sent to our rooms ready for your first appointment.
